This form is to be completed for the consideration of PMHA Scholarship Award. •••••Each year the Paris Minor Hockey Association awards one scholarships of $500.00 to a 3rd year Midget players. The Paris Minor Hockey Association encourages all high school graduating players (male and female) to apply. These scholarships are based on a point system. The recipients of these scholarships must meet the following criteria:••••• •••••• Applicants must be a graduating high school students as of September of the year application is submitted.••••••Applicants must reside in an area within the boundaries of the Paris Minor Hockey Association.•••••• Applicants must have registered with the Paris Minor Hockey Association for the previous season.••••••The applicant must be in good financial standing with the Paris Minor Hockey Association.••••••Applicant must be enrolled in a post-secondary educational institution. •••••The player is asked to complete the necessary application and file with PMHA by April 15 of the current year, along with an official high school transcript, an essay of approximately 500 words discussing your educational goals and any other comments which may aid the selection committee in its decision.•••••Coaching staff(s) are asked to solicit feedback and submit names of Players who meet the above criteria of this award. After reviewing all entries the PMHA Executive will select one winner.
